I've noticed this (or something very similar) on other 9a Fire escapes. I therefore suspect that it is actually a deliberate design feature, and can only assume that the nick in the escape wheel axle tube is something to do with the tooling arrangement for creating the hole for the shaft.

Fantastic! would love that in my collection. This is the most 'OOPSED' model in the range with seats missing, mis-struck base rivet, deformed steering wheel and reversed road wheels and now with glazing the wrong way round, amazing.

I would suspect that the Rolls Royce Factory was very displeased seeing all these OOPS Rolls Toys finding their way through Lesney Quality Control and then being sold to us commoners and also the general public Ghosty. Do you suppose the Lesney workers actually had some extra fun poking at the Rolls Marque while assembling these toys with such deliberate defects?
