I've never had anyone offer to pay when I've underestimated the postage!
I mostly sell on a site called etsy and if I didn't post to the US and Canada I'd be as well packing it in. Etsy has a system where you quote a postage price for the item, and another price if it is in with something else. This makes sense from a weight point of view, the only problem I have is when I sell an order of maybe 5 items which have a total value of £20 plus. 20 quid is my 'cut off' point - above that and I send with some kind of tracking, usually at my own expense. Below that and if it goes missing I refund out of my own pocket. If I applied tracking to every item I'd lose a lot of sales so reckon it's best to take the risk - it happens maybe 3 times a year. I would never ask a buyer to make up a shortfall as that would be like expecting them to pay for my mistake. I always refund overcharges and very rarely get thanked for it.
From a buyers point of view I'm sick of ebay sellers profiting on postage and have no hesitation in marking them down for it. There may be a few who do it through laziness, but I reckon most of them know fine what they are doing so don't even give them the opportunity to make it right, I'd rather hammer their feedback.
